How Much Do GLP-1 Medications Cost Without Insurance?
A dated snapshot of selected Wegovy and Zepbound self-pay offers, with the conditions that change the price and a checklist for comparing your total bill.
Educational content. Clinical review pending. How we prepare our guides
The answer depends on the medicine, presentation, strength and program you qualify for. A manufacturer’s self-pay offer can differ from a pharmacy’s cash price, an insured copay or a clinic’s total package.
Below are selected US manufacturer offers checked on October 7, 2026. They are medication-price examples, not guaranteed quotes, prescriptions or a ranking of treatments. They exclude any separately billed care unless the source explicitly includes it.
A dated manufacturer-price snapshot
| Product and program | Advertised amount | Important condition |
|---|---|---|
| Wegovy injection introductory self-pay offer | $199 per monthly fill | New eligible participants; two fills of the specified 0.25 mg and 0.5 mg presentations through December 31, 2026 |
| Wegovy injection ongoing self-pay offer | $349 per month for listed 0.25-2.4 mg presentations; $399 for HD 7.2 mg | Check the exact presentation and program terms |
| Wegovy tablets, self-pay | $149 at 1.5 mg; $199 at 4 mg; $299 at 9 mg or 25 mg per month | Price changes with tablet strength |
| Zepbound single-patient-use KwikPen, regular self-pay prices | $299 at 2.5 mg; $399 at 5 mg; $499 at 7.5 mg; $699 at 10, 12.5 or 15 mg per month | These terms concern KwikPen, not every Zepbound presentation |
| Zepbound KwikPen purchase offer, selected higher strengths | $449 per month at 7.5, 10, 12.5 or 15 mg | Initial offer followed by a refill-purchase timing requirement of 45 days; eligibility and other terms apply |

Lilly states that 2.5 mg is a starting dose, not an approved maintenance dose. Its page has separate pathways and terms for different insurance situations and presentations. Do not assume that a headline starting price is your ongoing cost. Lilly’s current terms.
Medication price is one part of the bill
Ask the clinician or service to itemize anything outside the medication offer: consultation, membership, follow-up, tests, dispensing, shipping and supplies. Some of these may be included, optional or not applicable. The purpose is to make the unknowns explicit.
Record the billing period too. “One month,” a quantity of doses and a 28-day supply may not line up with another provider’s calendar-month membership. A comparison becomes more useful when you put actual payment dates beside each amount.
The peptide cost worksheet gives you a consistent way to collect those details.
Check which savings route applies to you
Do not substitute a commercial-insurance copay offer for a self-pay quote. On manufacturer pages, offers for people with covered commercial insurance, people without coverage for the product and people paying cash can have different conditions.
Before paying, ask:
- Which exact offer is being applied to this prescription?
- Does it cover the prescribed presentation and quantity?
- Am I eligible, and what documentation is required?
- What will the next refill cost after any introductory period?
- Is a refill deadline or minimum purchase commitment involved?
- Which care fees are payable even if no prescription is issued?
These are questions to the provider, pharmacy and program administrator. Pephelp does not determine eligibility or process these offers.
Make a sustainable comparison
A useful budget shows the initial payment and several subsequent payments separately. Keep any price you cannot verify marked “unknown.” Save the dated quote and offer terms so you can understand a later change.
Price comparisons should also preserve product identity. A compounded preparation and an FDA-approved branded medicine should not appear as interchangeable line items just because they mention the same ingredient. Our compounding explainer describes the distinction.
This page covers two selected manufacturers, not every GLP-1 medicine, program or access route. Pephelp has no treatment pricing or dispensing offer of its own.
